OPVMC Referral Portal

Stay connected to your patients’ care with around-the-clock access to medical records through the OPVMC Referral Portal.

Keeping Your Team Connected

The portal gives referring veterinarians a convenient way to share information and receive updates from Orchard Park Veterinary Medical Center’s specialists and emergency department.

It complements our regular phone communication. You can also continue receiving information by fax or email, based on your preferences.

How to Use the Portal

Submit a referral. Log in and enter the referring veterinarian’s name, practice details, and client and patient information. Attach relevant medical records, radiographs, and lab results.

  • Receive updates. As your patient arrives and receives care, information is shared through your selected channels: fax, email, and/or the portal.
  • Access records anytime. Review information for all patients you refer to OPVMC, 24 hours a day, from a computer, smartphone, or tablet.
  • Follow your patient’s care. Available records include check-in and check-out details, medical notes, lab results, prescriptions, diagnoses, problems, and attachments.

Refer a Patient

Submit a referral and upload supporting records, including blood work results, X-rays, and other relevant documents.

Customize Your Settings

Select the notifications you’d like to receive and choose delivery by email, fax, or both.

Manage your preferences for patient check-ins, discharges, deceased patient notices, and care updates. Patient updates and records will still be delivered by your selected method, even if you don’t use the portal.

How the Referral Portal Supports Your Practice

  • Save time with convenient communication between your team and ours.
  • Stay informed with timely access to the medical information you need for ongoing patient care.
  • View records around the clock from a computer, smartphone, or tablet.
  • Send referrals and upload related documents in one convenient place.
  • Keep referral information organized to help reduce missing documents and repeat submissions.
  • Choose how you receive updates and records: through the portal, by fax, by email, or a combination of all three.
